Funeral Forms and Certificates

The following list summarises the forms required for different types of funeral arrangement. You can get more information about these forms and how and when to complete them in our guidance notes, available to download below.
 
If the body is to be cremated and has not been referred to the Coroner or, when referred, a Notification by the Coroner (Pink Form 100) has been issued.
 
  • Preliminary Application for Cremation (Pink for Family Chapel, Green for Main Chapel)
  • Registrar's Certificate (Green)
  • Application for Cremation (Form A)
  • Medical Forms (Certificates B & C)
  • Authority to Cremate (Form F)
 
If the body is to be cremated and a hospital post mortem has taken place and the results are known to the doctor who attended the deceased before death.
 
  • Preliminary Application for Cremation (Pink for Family Chapel, Green for Main Chapel)
  • Registrar's Certificate (Green)
  • Application for Cremation (Form A)
  • Medical Form (Certificate B)
  • Authority to Cremate (Form F)
 
If the body is to be cremated and a post mortem examination has been made or an inquest opened.
 
  • Preliminary Application for Cremation (Pink for Family Chapel, Green for Main Chapel)
  • Application for Cremation (Form A)
  • Coroner's Certificate for Cremation Form E
  • Authority to Cremate Form F
 
If the body is to be buried and death has not been referred to the Coroner.
 
  • Burial Authority Form
  • Registrar's Certificate (Green)
 
If the body is to be buried and there is an inquest.
 
  • Order for Interment
  • Coroners Order for Burial (Form 101)
