Employers Registration Form
If you intend to employ people in offices, shops or other premises, you are required to notify the enforcing authority beforehand, to protect the health, safety and welfare of people employed to work.
This is done by completing an OSR1 employers registration form and sending to the council, for offices and shops, or the HSE for some other types of business. You can download the council's form in PDF format below.
The definition of offices and shops includes catering premises open to the public - cafés, restaurants and public houses - and wholesalers. For more information go to Registering Your Business on the HSE website.
